Health History
Your medical and dental history provides valuable insights that help us deliver the safest and most effective care. Understanding your health allows us to assess risks for dry mouth, cavities, and ensure safe treatment when prescribing medications. When filling out your new patient forms, please include a complete list of your current medications and medical history so we can provide you with the best possible care.
Dental Insurance
As a courtesy, we bill your insurance company and track claims on your behalf. Please notify us of any changes to your plan to ensure accurate processing. While we work with your insurance provider, you are responsible for any fees charged by our office. Most insurance companies process claims within four to six weeks, and any remaining balance will be your responsibility.
Financial Policy
Payment is due at the time of service unless prior arrangements have been made. For your convenience, we accept cash, check, Visa, MasterCard, and Discover.
We also partner with CareCredit to offer flexible payment plans, making dental care more accessible. CareCredit allows you to pay for your dental care over time with convenient monthly payments. It can be used for a variety of treatments, including routine checkups, fillings, cosmetic procedures, and more. You can apply online or in our office, and once approved, you can use your credit line immediately to cover out-of-pocket expenses—ensuring you get the care you need without delay.
