What to expect…
Health History
An accurate medical and dental health record is essential because it can offer valuable insights to the dentist before initiating treatment. This knowledge helps us address dry mouth risks, cavities, and ensures safe treatment with any prescribed medications. When filling out your new patient forms, please provide us with a list of current medications and medical history. We believe that knowing more about your health enables us to provide you with the best dental care.
Dental Insurance
As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
Financial Policy
Payment is due at the time service is rendered unless other arrangements have been made in advance. For your convenience we accept cash, check, Visa, MasterCard or Discover. We have partnered with CareCredit for affordable payment plans. CareCredit offers no-interest and extended payment plans for qualified applicants.
